The best book I've ever read was a small paperback from the UK called, "Web Project Management". Far best investment beyond any college and techie books read. As for web design, I use photoshop to create a prototype. Next, I create an HTML document with a stylesheet and design the placeholders (elements) focusing on SEO.

I try to be practical. If I can start from a template, then that's what I do. I just make necessary modifications. But there are cases in which I start from scratch.
