As far as the forum is concerned I think forum topic, where the actual question is being asked is important. If someone is searching an answer of the question like it is in the topic it should be easy for someone to go through your forum.

Things I would say would be: (in addition to the op)
  • Animated GIFs (any if not lots)
  • Massive Images (as in 100's of kb in size)
  • Thumbnails (like fake where it is the full image just specified at a smaller size*)
  • Background images that make text hard to read
  • Bad colour usage (dark on dark or light on light etc)
  • Non-resizeable text

Can't think of any others atm... :P

* Just say an 800x600px image shrunk down to 200x150px instead of being recreated at that size (if you get what I mean)

For me a site that takes too long to load is not worth the time.
There are some sites with really nice graphics but it takes over 30 seconds to load.

30 seconds in internet time is forever. Only way to overcome to I am going to leave this site right now is to spend a whole lot on advertising and even then it is a long shot.

well ! i don't like
1) flash intro/messages and no way to skip,
2) heavy graphics
3) useless navigation system
4) no sign, where i'm and where to go next
5) layouts that too much stretch even in high resolution screens
6) unncessary signup requirements for even reading the content
7) when asking for personal info or email
8) beyond understanding fonts
9) difficult to find content and not getting results i need

If I see some flashy Contents, way too many links, massive photos are ok, I can disable it, and to get to one page. I have to travel thousand pages.... etc

6- The page doesn't really say what the page is about, and hence, the visitor cannot figure out whether he/she landed on a page he wanted to come visit.

On the internet, people are inpatient.
