Have you gone to hotscripts.com? I've spent a lot of time there myself looking around for different scripts and they have pretty much anything you can think of there. Some are free, some are not. I would imagine though that they probably have exactly what you are looking for, probably free too!

I like to suggest you that you just download any forum script which is available freely online and find in it. Almost now 99% scripts has this option to ban IPs.

you could simply have a database that everytime a php script is accessed, check for their IP if it exists in the ban_ip_table.

Nowadays lot of users use DSL connection which has dynamic ip so everytime someone restarts a modem or a router, there is different ip address. If you are planning to use it to ban users accessing your website, it is very tough to monitor.
