Adwords Campaign Going out of control?

I have been some serious problems lately with google adwords., It seems that the my total expenses are going beyond my specific limits and i am incurring expenses more than my estimates... I have heard that thing in the past but never faced it myself.. Any idea on what might have went wrong?
